When you receive care, you may need to pay out-of-pocket costs like copayments, coinsurance, or deductibles. If you see a provider or visit a facility outside your health plan's network, you might also face extra charges.

Out-of-network providers don't have contracts with your health plan. They may send you a bill for the difference between what your plan pays and what they charge — this is called balance billing.

You may experience surprise billing when you get an unexpected balance bill, especially in situations where you can't choose your provider, such as:

  • Emergency care
  • Treatment by an out-of-network provider at an in-network facility
