Mobile Clinic

Your health is our priority. As part of our commitment to your well-being, we offer primary care services in Huerfano and Las Animas counties through our Mobile Clinic. Below is the  schedule for your convenience: 

Primary Care Schedule: 

  • Aguilar: 9 am - 3 pm, First Friday of every month 
    • Aguilar Jr. Sr. High School 
    • 420 Balsam Ave. 
    • Aguilar, CO 81020 
  • Gardner: 9 am - 3 pm, First Thursday of every month 
    • La Clinica 
    • 2485 State Highway 69 
    • Gardner, CO 81040 

When calling to schedule, please mention that you are requesting an appointment for the  Mobile Clinic.
